Preparing for Your First Visit

We're excited to welcome you to our practice! To help ensure a smooth and efficient first appointment, please review the information below before your visit.

Complete Your New Patient Forms

To save time and help your appointment run smoothly, please complete your new patient forms before arriving.

You can access your forms by:

  • Using the secure link sent to you via text message or email
  • Visiting the Patient Forms section of our website 

Please note: Links sent by text or email remain active for 10 days.

If you have not completed your forms before your appointment, you must arrive 15–20 minutes early to complete the paperwork in the office.

Dental Records & X-Rays

To provide the most comprehensive evaluation possible, we ask that all new patients contact their previous dental office and request that their records be transferred to our office before their appointment.

We require a current Full Mouth Series (FMX) of X-rays or equivalent diagnostic images for your initial examination. Please have the following X-rays transferred if available:

  • Full Mouth Series (FMX)
  • Panoramic X-ray (Pano)
  • Bitewing X-rays
  • Other recent diagnostic radiographs, as appropriate

Records and X-rays can be forwarded to xrays@silversfamilydental.com.

Please note:

  • It is the patient's responsibility to ensure records are transferred from their previous dental office.
  • Records should be received before your appointment date whenever possible.
  • If diagnostic X-rays are not received prior to your appointment, there may be an out-of-pocket cost for obtaining updated images.

When requesting your records, ask your previous dental office to send the X-rays and records to both xrays@silversfamilydental.com and directly to you. This allows you to verify that the transfer was completed successfully before your appointment and can help prevent delays, rescheduling, or additional costs.

Types of Appointments

New Patient Establishing for Routine Care (Ages 18 and Older)

At your first dental appointment, we'll begin with a comprehensive oral evaluation with one of our doctors. This may include diagnostic X-rays, if needed, to assess your overall dental health.

During this visit, we will:

  • Review your medical and dental history
  • Discuss any concerns or symptoms you may have
  • Perform a comprehensive examination
  • Develop a personalized treatment plan based on your needs

Please note that after your initial exam appointment, any recommended treatment and/or routine cleaning will be scheduled for a future visit.

New Patient Establishing for Routine Care (Under Age 18)

At your child's first dental appointment, they will meet one of our friendly dental hygienists who will perform a cleaning and obtain any necessary X-rays to evaluate for decay and monitor dental development.

During this visit, we will:

  • Review your child's medical and dental history
  • Complete a professional dental cleaning
  • Take diagnostic X-rays, as needed
  • Perform a comprehensive oral evaluation with one of our doctors
  • Apply topical fluoride when appropriate to help protect against tooth decay

New Patient Emergency Appointment (All Ages)

If you are experiencing dental pain, swelling, trauma, or other urgent concerns, our scheduling team will arrange an emergency evaluation with one of our doctors.

During an emergency visit, the dentist will:

  • Take any necessary X-rays
  • Perform a focused examination related to the area of concern
  • Diagnose the problem
  • Discuss treatment recommendations and next steps

Treatment will be scheduled according to your individual needs and the nature of the emergency.

In some cases, emergency conditions may require referral to a dental specialist for prompt treatment.

Important Appointment Requirements

To keep your scheduled appointment, please ensure that:

  • Your appointment has been confirmed
  • Your requested dental records and X-rays have been transferred
  • Your new patient forms have been completed

Appointments that remain unconfirmed, are missing required records/X-rays, or have incomplete new patient paperwork may be subject to cancellation or rescheduling.
